Graduate Admission Forms

Change of Address	Logon to the Automated Student Access Program (ASAP) to change your address. Under Personal Information Tab, click Update Addresses and Phones	Must login to ASAP system
Change of Classification	Logon to the Automated Student Access Program (ASAP) to change your address. Under Student Services Tab, click Graduate School, select Request for Change of Major or Degree Information. Complete this request if you would like to change your classification from Special Graduate or Non- Degree to Degree Seeking.	Must login to ASAP system
Change of Concentration	Logon to the Automated Student Access Program (ASAP) to change your address. Under Student Services Tab, click Graduate School, select Request for Change of Major or Degree Information. Complete this request if you would like to change your concentration.	Must login to ASAP system
Change of Major	Logon to the Automated Student Access Program (ASAP) to change your address. Under Student Services Tab, click Graduate School, select Request for Change of Major or Degree Information. Complete this if you are currently enrolled as a graduate student and wish to change to another graduate program. There is a $5.00 fee associated with this service.	Must login to ASAP system
Change of Catalog	Logon to the Automated Student Access Program (ASAP) to change your address. Under Student Services Tab, click Graduate School, select Request for Change of Catalog. Complete this if you are currently enrolled as a graduate student and wish to change to another catalog.	Must login to ASAP system
Deferral Form (Master's Students Only)	Logon to the Automated Student Access Program (ASAP) to change your address. Under Student Services Tab, click Graduate School, select Request for Application Deferral. Complete this if wish to defer your online application.	Must login to ASAP system
Graduation Application	Logon to the Automated Student Access Program (ASAP) to apply to graduate from UTSA. Under Student Services Tab, click Graduation, select Graduation Application.	Must login to ASAP system
Military Duty Statement	If you or your spouse are stationed at a Texas military base, you can apply for in-state tuition by completing this form.	Military Duty Statement (PDF Format)

Release of Degree Information	Master's and doctoral candidates should request a Letter of Degree Completion from their Graduate Advisor of Record or the College graduate advisor up to the time the semester ends.	Release of Degree Information (PFD Format)
Residency Core Questions	Use this residency questionnaire if applying for Fall 2006 and later. Students applying to Texas Higher Education Coordinating Board rule 21.731 requires each student applying to enroll at an institution to respond to a set of core residency questions for the purpose of determining the student’s eligibility for classification as a resident. Non- resident students currently enrolled may use this form to apply for consideration for re-classification to a Texas resident for tuition purposes. Requests for reclassification must be received prior to the official census date of the term which you are requesting reclassification. The determination of residency classification for admission and tuition purposes is governed by legislative statues found in the Texas Legislature and rules and regulations promulgated by the Texas Higher Education Coordinating Board.
